Paperwork for initial ADD/ADHD evaluation
Prior to making an appointment for an evaluation for attention deficit disorder (ADD) or attention deficit hyperactivity disorder (ADHD), please complete and submit the required assessment forms. Many of the PDF forms can be filled out online and printed.
You’ll need Adobe Acrobat Reader to view the PDFs. Download Adobe Acrobat Reader for free.
Parents should complete the following forms:
- Child Behavior Checklist (1-5 yrs)
- Child Behavior Checklist (6-18 yrs)
- Parent Questionnaire
- NICHQ Vanderbilt Assessment Scale for Parents
- Conners’ Global Index
Teachers should complete the following forms. If the child has several teachers, all should fill out the assessments:
- Caregiver / Teacher Report Form (1 – 5 yrs)
- Teacher’s Report Form (6 – 18 yrs)
- NICHQ Vanderbilt Assessment Scale for Teachers
Paperwork for follow-up ADD/ADHD appointments
Prior to your child’s follow-up appointment for attention deficit disorder (ADD) or attention deficit hyperactivity disorder (ADHD), or as instructed by your physician, please complete the following forms and bring them to your child’s appointment. You may also fax the forms to us ahead of time to (469) 671-5437.
Parents should complete the following:
- Vanderbilt Follow-Up
- Conners’ Parent Version
- Progress Update (for grades K-5 only)
- Progress Update (for grades 6-12 only)
Teachers should complete the following:
Please note: We’re not able to perform an ADD/ADHD exam and a well exam in the same office visit. Separate appointments will need to be scheduled.
